Step 1: Submit Your Application
In order to enroll in PS-S, you must submit a completed application.
You can obtain an application using any of the following methods:
- Download an Application
Download our Program Application
(.PDF FORMAT). You will need Adobe Acrobat Reader in order to view the application.
Download ADOBE READER.
- Pick up an Application
Pick up
an application at the Loveland or Fort Collins
PS-S office.
- Call to Request an Application
Call (970)
635-5912 and request that one be mailed to you.
- Email to Request an Application
Email PS-S at PSS@FRII.COM.
The application is a simple, one page, two sided form that asks for
basic information. In addition to the completed application, PS-S also
requires a letter of recommendation from someone who can comment on your
strengths and desire to become self-sufficient. This individual could
be a counselor, employer, minister, teacher, etc., but not a family member.
Step 2: Wait for an Opening
When openings in the PS-S program become available, applicants
are invited to attend an orientation based upon their date of application.
It is very important that
you keep us informed of any change in address once you apply. Otherwise
we are unable to notify you of the orientation.
At the orientation you
will learn more about PS-S and decide if the program is a
good match for you. If you are still interested, an
interview will be scheduled so that PS-S can get to know you and understand
your needs.
If we all agree that this is a good match, you will be assigned
to work with a PS-S Advisor and begin your enrollment in the program.
Step 3: Participate in the Program
Once enrolled, the average length of stay in the program
is two to three years. Some participants may need more or less time.
You can decide to leave the program at any time. Participants can be
removed for failing to follow through with their agreed upon contract.
Contracts can be revised by mutual agreement of both parties.
